ADMISSIONS

It all starts with a phone call to the intake and assessment department at 888-629-3471. Brett Miller, our Director of Intake or Michelle Adams-Rabe will be happy to discuss your child’s need for residential placement. They will begin the process by asking you about your child’s prior treatment, clinical history and family information to assure that we are the most appropriate level of treatment for your child.

We always invite you to tour our facility and meet our staff before making a decision to place your child with us. Tours are given Monday-Friday 9:00-4:30 CST, special arrangements can be made for weekend and after-hours tours.

Some indicators that residential treatment is necessary include but not limited to:

One or more acute care hospitalizations
• A history of multiple failed treatments
• Harmful to self or others
• School failure
• Co-occurring substance abuse
• Patterns of persistent disruptive behavior
• Poor social skills/behavior


A thorough assessment is provided to the family at no cost. At the time of tour and assessment, please bring the following documents with you:

• Immunization Records
• Copy of Individual Education Plan (IEP)
• Copy of Birth Certificate
• Custody Records (copy of divorce decree, custody status, etc.)
• Copy of Social Security Card
• Copy of Parents Drivers License and Insurance Cards (As Applicable)
• Copy of Insurance Card (If Applicable)
• Pharmacy Card
• Copy of Medicaid Card (If Applicable)
• Name and Address of Child’s School and a Contact Name
• School Records
• Name and Address of Pediatrician, Psychiatrist, Therapist (Or Other Treatment Providers)
• Copy of Case Plans
• Copy of Current Psychological Testing (If applicable)


An admission time and date will be set at the time of assessment; however it is sometimes possible to admit the same day of the assessment and this can be discussed with Brett and Michelle during your initial phone call.

Inquiry calls made after business hours (8:00-4:30 CST) will be routed to our evening staff but will be followed up the next business day by Brett or Michelle. Free confidential consultations are always provided. If you prefer, you may fill out the online information request and a representative from the Intake Department will contact you within 24 hours.
